How to Find Out How Many Traffic Violations Are on Your Driving Record in Illinois
- 1). Fill out a Driving Record Abstract Request Form. The form can be found online via the Illinois Secretary of State's website. In Section I, enter your name, driver's license number, birth date, and gender. In Section II, enter your name and address, your driver's license number, and check "yes" to indicate that you are seeking your own record. In Section IV, check the first box to indicate that you are the person whose driving record is sought. In Section V, sign and date the form. You will need to have your signature notarized if you plan to mail in the form.
- 2). Visit your nearest Secretary of State facility, if you are in Illinois. You will need to pay $12, the fee as of the time of publication. Credit and debit cards are not accepted. If you choose this option, you can get your driving record immediately.
- 3). Mail in your form, if you cannot or don't want to visit the Secretary of State facility. Send your Driving Record Abstract Request Form, and your $12 payment, to:
